Public Records Request Form

The California Public Records Act (Government Code Section 6250 et. seq.) requires the City to respond to a request for public records within 10 calendar days. The City Clerk's Office will respond to the request at the earliest opportunity.

Please note: Certain records are exempt from mandatory disclosure under the Public Records Act and should be requested from the City Attorney's Office. Public records requests are a matter of public record.

By submitting this request, I/we agree to pay the City of Chula Vista at the rate of 15 cents per page (10 cents per page for the State-mandated filings) at the time of receipt of copies by myself or my representative. If I/we request to have records faxed, I/we agree to make payment in advance at the rate of 40 cents per page to numbers within San Diego County and 50 cents per page to numbers outside San Diego County.
