The following information must be submitted with your application prior to the approval of your permit. Applications will be accepted at the Chula Vista Police Department on a walk-in basis on Tuesdays from 9:00 – 11:30 AM or 1:00 – 4:00 PM or by appointment by calling 619-691-5244.

Please be aware that an incomplete application submitted by mail will cause delays in the processing of your permit.
Required Documents & Fees
- Completed Police Controlled License
- Signed Statement of Understanding
- Copy of current City of Chula Vista business license
- Payment of $322.00 (Checks and money orders are made payable to the City of Chula Vista)
All fees are non-refundable.
Your Police Controlled License is valid for one year from the date of issue.
